Neighborhood Coalition for Shelter (NCS) helps New Yorkers who have a history of homelessness, often compounded by mental illness or substance use, to achieve their highest level of independence. We were established in 1982 by faith and community leaders on Manhattan’s Upper East Side who believed that homelessness is the responsibility of the entire community. More than 40 years later, we are still guided by the principle that neighbors should help neighbors.
Today, NCS offers an array of groundbreaking housing and supportive services, including counseling, substance use treatment, vocational and educational programs, and employment support, and we are poised for further growth.
NCS provides New Yorkers who are homeless or at risk of homelessness with a home, a community, and access to the services that they need to thrive.
ABOUT THE PROGRAM:
Louis Nine House (LNH), located in the Bronx, has 46 studio apartments for young adults (aged 18-25) who are homeless or have aged out of foster care. Many of our tenants have experienced multiple foster care placements, emotional trauma, neglect and/or abuse, and many also struggle with mental illness and substance use. We provide affordable housing and an integrated continuum of support services designed to help young adults achieve stability, maintain sobriety, and work toward the self-sufficiency needed to obtain and sustain permanent housing and independence. Through case management; onsite psychiatric services; and our vocational and educational program, residents are supported to advance their personal development, pursue their education, identify career interests, acquire vocational skills, and secure employment.
POSITION SUMMARY:
The Senior Case Manager plays a critical role in providing comprehensive case management services to individuals experiencing homelessness or housing insecurity. They work closely with clients to assess their needs, develop individualized service plans, and connect them with resources and support systems to achieve stability and self-sufficiency. The Senior Case Manager also supervises and supports 2 case managers, ensuring the delivery of high-quality services and adherence to organizational policies and procedures.
RESPONSIBILITIES:
Case Management and Support
· Oversee and manage a caseload of residents, ensuring their needs are met effectively and efficiently.
· Ensure that residents work with their case managers to develop and implement individualized action plans that includes time-defined goals based on their specific service needs to develop life and work skills.
· Monitor and evaluate residents' progress towards goals, adjusting plans as necessary to ensure optimal outcomes.
Client Advocacy
· Advocate on behalf of residents to ensure they receive appropriate services and resources.
· Act as a liaison between residents and service providers, facilitating communication and resolving any issues that may arise.
· Identify and connect residents with community resources, such as housing assistance, financial assistance, legal assistance, healthcare services, vocational training, and support groups.
· Assist residents in accessing and navigating available resources, providing guidance and support as needed.
Team Collaboration
· Work collaboratively with arts coordinator and residents to develop activities that will Collaborate with interdisciplinary teams, including social workers, healthcare professionals, therapists, and other stakeholders, to coordinate services and support for residents.
· Participate in or facilitate case conferences, team meetings, training sessions and other forums to discuss client progress and develop coordinated plans of care.
Documentation and Record Keeping
· Ensures that junior case managers maintain accurate and up-to-date tenant records, documenting assessments, action plans, progress notes, and any relevant communications or interactions.
· Ensure compliance with regulatory requirements and organizational policies regarding confidentiality and record keeping.
Crisis Intervention
· Provide crisis intervention and support to residents in times of emergency or heightened need, collaborating with other professionals and agencies as necessary to ensure client safety and well-being.
Supervision and Leadership
· Provide guidance, mentorship, and supervision to 2 junior case managers, assisting them in their professional development and ensuring quality service delivery.
· Serve as a resource and subject matter expert, providing consultation and support to colleagues as needed.
Continuous Improvement
· Participate in ongoing training and professional development activities to stay current with best practices, trends, and developments in the field of case management.
· Identify opportunities for process improvement and quality enhancement within the organization, contributing to the overall effectiveness and efficiency of services.
Reporting and Evaluation
· Prepare reports and presentations on client outcomes, program effectiveness, and other relevant metrics, for internal use or external stakeholders such as funding agencies or regulatory bodies.
· Participate in program evaluations and quality assurance activities, providing feedback and insights to inform program planning and improvement efforts.
QUALIFICATIONS:
· Bachelor's degree, master’s preferred, in social work, psychology, counseling, or a related field.
· Previous experience in case management, preferably working with a SMI/SED population of young adults.
· Strong understanding of the developmental needs of young adults/at-risk youth. Sensitivity to ethnic and cultural diversity and knowledge of and commitment to working from a strength-based youth-development perspective.
· Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations.
· Experience supervising or mentoring staff members.
· Knowledge of community resources and social service agencies in the local area.
· Ability to work independently and as part of a team, with a commitment to client-centered and strengths-based approaches.
· Proficiency in Microsoft Office Suite and case management (e.g., Awards) software systems.

Salary: $50,000 - $60,000
Final pay rate will be determined based on various factors which may include internal equity, work experience, skills, knowledge, and/or education and/or certifications.
